Lev Pediatrics protects your child's protected health information (PHI) in accordance with HIPAA. We use PHI to provide care, coordinate with specialists, process payment, and fulfill legal obligations.

We will never sell your information. You have the right to access records, request corrections, and receive an accounting of disclosures. After-hours messages and care coordination may be sent through secure, HIPAA-compliant channels.

I authorize Lev Pediatrics and its providers, including Rivkah Franklin, PA-C, under the supervision of a licensed physician, to provide medical care to the child named above. This includes examinations, routine laboratory tests, immunizations, prescribed medications, and other treatments deemed medically necessary.

I understand I may decline any specific treatment and discuss alternatives. For emergencies, I authorize transport to the nearest appropriate facility.

I consent to receive care via secure video and messaging when clinically appropriate. I understand telehealth has limitations, and the provider will recommend in-person care when needed. Communications are encrypted and HIPAA-compliant.
